Analisis Penerapan Rekam Medis Elektronik Metode Doctor’s Office Quality Information Technology (DOQ-IT) di Puskesmas X

Digital transformation of healthcare services through the implementation of Electronic Medical Records (EMR) is a national policy that must be adopted by all healthcare facilities, including primary health centers; however, its implementation at the primary care level still faces various challenges. This study aimed to analyze the implementation of EMR at Primary Health Center X using the Doctor’s Office Quality–Information Technology (DOQ-IT) framework. A descriptive qualitative design was employed and conducted at Primary Health Center X with four informants selected through purposive sampling, consisting of one key informant, the head of the medical records unit, and three main informants, namely a medical records officer, a nurse, and a midwife. The variables examined included human resources, organizational culture, leadership governance, and health information technology infrastructure. Data were collected through in-depth interviews, field observations, and document review, and were analyzed thematically. The findings indicated that EMR implementation at Primary Health Center X has not been optimal, with major barriers related to inadequate human resource readiness and competency, compounded by limited continuous training, resistance to technological change within the organizational culture, weak leadership supervision and evaluation, and insufficient information technology infrastructure. In conclusion, EMR implementation policies at the primary care level need to consider overall organizational readiness, as successful implementation depends not only on technological availability but also on human resources, organizational culture, and governance aspects.
