Abstract

 Schizophrenia is a severe mental disorder that may cause disturbances in perception, thought processes, emotion, and behavior. One common nursing problem in patients with schizophrenia is sensory perception disturbance in the form of auditory hallucinations. This case study aimed to describe psychiatric nursing care for a patient with schizophrenia experiencing sensory perception disturbance: auditory hallucinations. This study used a descriptive case study design involving one 26-year-old male patient with a medical diagnosis of schizophrenia. Nursing care was provided for 3 × 24 hours through assessment, nursing diagnosis, planning, implementation, and evaluation. The main nursing diagnosis was sensory perception disturbance related to auditory disturbance, accompanied by delusion and ineffective coping. The intervention focused on hallucination management, including identification of hallucination content and triggers, education, rebuking hallucinations, medication adherence, talking with others, positive activities, music therapy, and positive reinforcement. The results showed that sensory perception disturbance was partially resolved. The score for verbalization of hearing voices improved from 2 to 3, hallucinatory behavior from 2 to 4, daydreaming from 2 to 4, and pacing from 2 to 4. The patient appeared calmer, more cooperative, was able to rebuke hallucinations independently, and used music and conversation as distraction techniques. Hallucination management may help patients gradually improve their ability to control auditory hallucinations.
