Low consumption of fruits and vegetables by adolescents in Indonesia is a public health issue that can increase the risk of various non-communicable diseases in the future. Sheet-based health education is one of the methods that can be used to increase teenagers' awareness of the importance of consuming fruits and vegetables. This study aims to determine the effectiveness of sheet-based education on changes in perceived vulnerability and perceived severity of fruit and vegetable consumption behavior among teenagers at SMP Al-Amanah in 2026. This study uses a quantitative approach and employs a one-group pretest-posttest pre-experimental design. The purposive sampling method was used to select samples for the research. Pre-test and post-test questionnaires were used to collect data. To analyze the results, the average scores of the two components of the Health Belief Model before and after the intervention were compared. The research results show that after the counseling, the vulnerability perception score increased by 6 points from 17 to 23, and the seriousness perception score increased by 22 points from 11 to 33. The largest increase occurred in the seriousness perception component, indicating that the leaflet helped students understand the serious impact of insufficient fruit and vegetable consumption. Thus, the leaflet proved effective as a health promotion medium because it increased vulnerability perception and teenagers' awareness of the importance of consuming fruits and vegetables. With the support of family and school, continuous health education is expected to help people improve their consumption of fruits and vegetables.
